Trinity’s Student Health Insurance Plan

Please Note! All undergraduate students enrolled in 6 credit hours or more are required to have health insurance. Students can have their own coverage outside of Trinity or select one of the Trinity plans. All undergraduate students who do not plan to enroll in one of the student insurance plans through First Agency must provide proof of their insurance coverage.

The Student Health Plan is through First Agency of Kalamazoo, MI. Please read the plan brochure (available in HR) very carefully. The information on this sheet is NOT intended to be a substitute for the information in the brochure.

What type of insurance program is it?

The insurance plan offered is a 70/30 co-pay plan. This plan has a deductible of $250.00 per person and $500.00 maximum for a family, and the plan covers a maximum of $15,000.00 per claim. The student plan is not a group plan like an HMO or PPO, but rather an individual plan.

What doctors and hospitals can I use?

The plan does not consist of a network of doctors, so you can go to any doctor or specialist anywhere in the world without a referral. Prescriptions are covered at the respective plan co-pays. What is the cost for the coverage?

On the enrollment card, premiums are listed individually for student, spouse, and each child. The cost is further broken down by age for those under 25 and those 25 and older. In the fall, you will have the option to sign up for the annual coverage and your account will be billed for the full premium. Please refer to the brochure for the current premium rates.

Who can enroll?

Any student registered in a program at Trinity is eligible to enroll. Coverage is available for spouses and dependents as well. NOTE: All International Students on an F-1 visa, including spouses and children, must have insurance coverage.

How do I enroll?

When you complete the Billing Information Form indicating the plan you wish to have, you must also return the completed Student Insurance Enrollment Card found in the brochure. Trinity will not sign you up for insurance without the Enrollment Card. Do not send your payment to First Agency, Inc. Your student account will reflect the charge.

What are the deadlines to enroll or make changes to my coverage?

Fall 2008 semester – September 20, 2009
Spring 2009 semester – January 28, 2009
Summer 2009 (May Term) – May 20, 2009

What is the cancellation policy for my coverage?

We allow students a full refund if coverage is dropped on or before the dates above. The annual plan period is August 15, 2008, to August 25, 2009.

How do I submit my medical bills for payment?

A claim form needs to be completed and submitted to First Agency for each claim. Claim forms are available in the Human Resources Office. Please be sure to complete both sides of the claim form. First Agency’s mailing address is on the top of the claim form. (For further information, follow the claim procedures on the back of the brochure)
